  • Visit one of our offices Monday – Friday 8 AM – 5 PM and make a payment in person with cash, check, or credit card.

  • Make a payment by check or credit card by mailing it with your bill stub to:

We are committed to ensuring that our services are affordable for those in need of financial assistance. Patients will not be denied services based on their inability to pay; based on whether payment for those services will be made under Medicare, Medicaid, or CHIP; or based on a patient’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.  We offer a discounted/sliding fee schedule available based on family size and income. We encourage you to discuss your options with a Patient Service representative by calling (800) 342-5653 Monday – Friday 8 AM – 5 PM or in person at one of our offices.

Our policy is to provide financial assistance for any eligible patient unable to pay for services.

We want you to know the cost of covered services before receiving care, so you are prepared, involved, and informed.

We provide a financial assistance program for any eligible patient that is unable to pay for services.

By law, we are required to provide patients with an estimate of their bill before those items or services are provided.

If you are a patient covered by Medicare, a Fee Schedule provides a comprehensive list of fee maximums used to reimburse a physician and/or other providers on a fee-for-service basis.
